Pay for 2010 Conference

By Mail
Step 1. Make your check or money order payable to KSA.

Step 2. Select the amount:

	Single	Family of 3 or More
KSA Members (by October 30)	$30	$75
Non-KSA Members (by October 30)	$40	$100
After October 30	$50	$120

Step 3. Be sure to write your name in the memo line.

Step 4. Donations are always welcome; so feel free to include a donation in addition to your conference registration.

Step 5. Mail your check or money order to:
KSA, P O Box 4148, Frankfort, KY 40604-4148.
